Napoli, 1414. Costanza Calenda, figlia di un famoso medico di Salerno, ha sempre sognato di seguire le orme del padre ed esercitare l’arte medica. Tuttavia, sa perfettamente che il suo destino è quello di sposarsi, in un mondo che vuole le donne soltanto mogli e madri.
Anche il padre, Salvatore, è di questo avviso, ma quando si reca alla corte di Napoli per curare il re in punto di morte, le consente di assisterlo. Per Costanza è l’occasione per mostrare a lui e alla futura regina, Giovanna II, i rimedi e le cure apprese di nascosto nella bottega di uno speziale. Giovanna ne rimane talmente colpita, che insiste perché la ragazza studi per diventare il suo medico personale. Ma poiché alle donne non è permesso frequentare l’università, Costanza dovrà proseguire gli studi in un convento a Bologna, lontana da casa e soprattutto dal giovane Baldassarre Santomango, l’affascinante nobile salernitano che le ha colpito il cuore.
Terminati gli studi, Costanza torna a Napoli per restare accanto alla sovrana; ne diventa l’amica e la confidente, scoprendo che Giovanna è circondata di nemici che tramano contro di lei. Nel frattempo Costanza ritrova Baldassarre e si accorge di amarlo più che mai. Forte del suo sostegno, la giovane fa di tutto per difendere la regina dagli intrighi di corte, e lei in cambio le permette di accedere all’Università di Napoli: potrà discutere la tesi in medicina, e diventare la prima donna laureata al mondo

Naples, 1414. Costanza Calenda, the daughter of a famous physician from Salerno, has always dreamed of following in her father’s footsteps and practicing the healing arts. Yet she knows all too well that her fate is to marry, in a world that sees women only as wives and mothers.
Her father, Salvatore, shares this belief—but when he is summoned to the royal court of Naples to treat the dying king, he allows Costanza to accompany him. For Costanza, it’s the chance to prove herself to him and to the future queen, Joanna II, by demonstrating the remedies and treatments she secretly learned in an apothecary’s workshop. Joanna is so impressed that she insists the young woman pursue her studies to become her personal physician. But since women are forbidden from attending university, Costanza must continue her education in a convent in Bologna—far from home, and far from the young nobleman Baldassarre Santomango, the charming Salernitan who has captured her heart.
After completing her studies, Costanza returns to Naples to serve the queen. She becomes Joanna’s friend and confidante, soon realizing that the sovereign is surrounded by enemies plotting against her. Meanwhile, Costanza reunites with Baldassarre and discovers that her love for him is stronger than ever. With his support, she does everything in her power to protect the queen from court intrigues—and in return, Joanna grants her permission to attend the University of Naples, where she will defend her thesis in medicine and become the first woman in the world to earn a degree.
